School forms provide essential information for the school to help your child achieve their full academic potential. These forms are also critical to ensure that your child’s needs are accommodated for.

Common forms that schools may require:

  • Health Assessment Forms

    Most schools would require their students to complete an annual physical evaluation with their pediatrician/medical provider. With the annual health assessment form, the medical provider can communicate the latest health status of the student to the school. This is useful in identifying health concerns that may impact the student’s school experience.

  • Medication and Medical Treatment Forms

    These forms provide vital information regarding diagnosis, medication, and any possible side affects associated with the medication/treatment. The most common out of these forms are the Anaphylaxis Action Plan and Asthma Action Plan.

  • Dietary Accommodation Form

    Students may have medical conditions impacted by their diet (examples: celiac disease & diabetes). Schools may have these forms to prepare for the appropriate meal for your child to ensure that they are safe and healthy. .

  • Diabetes Medical Management Plan

    While some students are able to manage their diabetes independently, those who are newly diagnosed may need aid in many aspects of their diabetes care. As a result, the doctor may have particular recommendations for the school to be able to ensure that the student’s needs are optimally fulfilled
